Electronic remittance advices (ERA 835s) were designed to replace paper EOBs. That transition remains incomplete. Many commercial payers continue to send paper. Medicare Advantage plans vary depending on the carrier. Workers’ compensation and auto insurers nearly always mail physical documents. A typical multi-specialty practice receives EOBs across at least four different channels: electronic 835, emailed PDF, faxed page, and mailed paper.
Paper creates a bottleneck at the data entry stage. Whether the EOB originates from Blue Cross, Aetna, Cigna, Medicare, or a Medicaid MCO, someone must read the document, identify the relevant fields, and key them into the practice management system. Faxed EOBs compound the difficulty because thermal paper degrades, fax resolution is poor, and multi-page faxes occasionally arrive out of sequence.
Scanning addresses the storage problem but leaves the data problem unsolved. A cabinet full of scanned PDFs is still just a collection of images. 200 DPI is the minimum for reliable results. 300 DPI is ideal. Faxed documents at standard resolution still work but may show lower confidence on small text like remark codes. Thermal paper that has faded should be rescanned at higher contrast before upload. Confidence scores on every field help you identify which documents need a second look.
