Electronic remittance advices (ERA 835s) were supposed to eliminate paper EOBs. That has not happened. Many commercial payers still send paper. Medicare Advantage plans vary by carrier. Workers’ compensation and auto insurers almost always mail physical documents. A typical multi-specialty practice receives EOBs in at least four different formats: electronic 835, emailed PDF, faxed page, and mailed paper.
Paper creates a bottleneck at the point of data entry. Someone has to read each EOB, identify the relevant fields, and key them into the practice management system. Faxed EOBs add another layer of difficulty because thermal paper fades, fax resolution is low, and multi-page faxes sometimes arrive out of order.

200 DPI is the minimum for reliable results. 300 DPI is ideal. Faxed documents at standard resolution still work but may show lower confidence on small text like remark codes. Thermal paper that has faded should be rescanned at higher contrast before upload. Confidence scores on every field help you identify which documents need a second look.
